SOCCER REGISTRATION INSTRUCTIONS

There are several items that are required to properly register your child with CYSA and RBYSL:

  • CYSA Registration Form (CYSA Membership Form 1601) and appropriate fees.
  • 1×1-1/2″ Color photo of the player with their name written on the back.

If a child is 8 years old or older, before 08/01/2010, you must provide a photo of the child with your registration application. Pictures must be current.

These photos will be laminated onto the player’s playing league pass – please do not submit over-sized pictures - as they do not fit correctly onto the space provided on the player pass. Do not submit photos for children born after the above date.

  • An official county or state issued birth certificate, passport, and alien registration card issued by the United States Government, a current driver’s license, or a current identification card issued by the Department of Motor Vehicles.
  • NOTE: If your child played in the spring 2010 or fall 2010 seasons, and you submitted proof of age, you are not required to submit it again.
  • HOSPITAL ISSUED CERTIFICATES, BAPTISM CERTIFICATES, OR OTHER RELIGIOUS CERTIFICATES WILL NOT BE ACCEPTED AS PROOF OF AGE.

All MAIL-IN registration must include the above documentation, and the appropriate registration fees. Incomplete packets will NOT be processed.
